OK, you mean background transfers. For background transfers some confirmations are by default suppressed. Please read documentation.

OK upload or download or both? Can you post a log file showing such transfer? And screenshot of Environment tab of preferences dialog?

standard FTP - and yes - happens for every file.

What protocol? Does it happen for any file? Or under some circumstances only?
Note that for technical reasons, WinSCP cannot ask for overwrite confirmations of files inside uploaded directories with SCP protocol. Is it your case?

WinSCP 4.1.6 (and all before) doesn't ask me if i really sure to overwrite a file - but in my options is all fine.

Any hints?
